We would like to ask you questions about your experiences supporting a person with an intellectual disability, autism, or other developmental delays.

This project is about understanding what good communication at hospital looks like and ways that healthcare workers, (like doctors, nurses, domestic support staff, social, and allied health workers) can communicate better with you and your supporters.

These questions will help us understand what has happened when you have supported a person with a disability from admission to discharge. We would like to know what worked and things that can be done better.

Before you complete the survey below, please ensure you have completed the consent form here.

DARE Disability Support and Nepean Blue Mountains Local Health District are working together to discover what good communication in hospital looks like for people with communication disabilities, in particular, people with intellectual and developmental disabilities.

This can be about:

  • The way that healthcare workers speak to the person you support,
  • How you felt when talking to healthcare workers, and
  • How important information about the person you supported was used.

We would like you to tell us about your observations and share your ideas with us so we can make communication better.

By filling in the consent form below, you’re signifying that you’re happy for us to use your survey response in our research.
